The TAPS questionnaire is designed to help you assess habits and behaviors related to substance consumption, including over-the-counter medications, prescription drugs, illegal drugs, and narcotics. This tool provides preliminary insights into patterns of substance use and offers an indication of potential risk for a Substance Use Disorder (SUD).

Please Note: This questionnaire is not a substitute for professional medical advice or a formal diagnosis. Its purpose is to prompt self-reflection and help you determine if further assistance is necessary.
